Nationwide PPO coverage is still available in West Virginia. Here's how plumbers approaching their late fifties in the Kanawha Valley compare plans when a spouse needs regular cardiology appointments.
Hi, I'm Sam Jaber. I'm licensed to broker health insurance in West Virginia. That's the only insurance I handle, and I work with clients entirely over the phone.
Imagine a 58-year-old Charleston plumber who's spent twenty years running a one-truck operation. His days take him from aging houses in the East End and South Hills to Kanawha City rentals, with jobs stretching through the valley between St. Albans and Marmet. He's replacing galvanized pipes and cast iron, dealing with flooded sump pumps after storms, and squeezing water heaters into cramped basements. His wife is 56 and handles the books. She survived a heart attack two years back, takes several medications every day and visits her Charleston cardiologist every few months. They're both still working hard, with no plans to ease up.
One thing I want to make clear: nationwide PPO coverage hasn't disappeared from West Virginia. I can get a quote for a couple living in Charleston.
Private nationwide PPO coverage involves a medical review before the insurer sets your price. With this couple, I'd need to understand the wife's heart history before deciding whether it's a good fit. Marketplace coverage works differently. For 2026, Kanawha County offers 37 medical plans through 2 insurers, including 15 PPOs and 22 HMOs. All must cover her regardless of her medical history, without charging extra because of it. There's also an interesting price difference: for a 40-year-old paying before credits, the lowest-priced silver PPO is 3.5% below the lowest-priced silver HMO. In fact, that PPO is the least expensive silver plan available in the county.
West Virginia premiums continue to climb. According to KFF, the state's average benchmark premium increased 16.8% between 2025 and 2026. For spouses in their late fifties, those costs make premium credits especially valuable. Eligibility depends on projected household net income for the coverage year. A plumber starts with business receipts and subtracts expenses such as vehicle costs, gasoline, materials, fixtures, equipment, licenses and liability coverage.
Before allowing your current coverage to renew automatically, take another look at your choices. Enrollment for 2027 coverage runs November 1 through January 15. To begin coverage January 1, you need to choose by December 15.
For this couple, the medical details should drive the choice. I'd begin by identifying her cardiologist, the hospital that handled her heart attack and the names and dosages of all her prescriptions. Then we'd verify how each plan covers them. Two similarly priced silver policies might handle the same cardiac medication quite differently. If her doctor refers her to a specialist in Columbus or Pittsburgh, a PPO can contribute toward care outside its network. An HMO generally won't cover that care unless it's an emergency.
Next step: Give me the cardiologist's name along with her full medication list and dosages. I'll review how each Kanawha County plan covers them.
At their ages, a small change in household income can mean a substantial difference in premium credits. That's why the estimate needs attention. Suppose a busy spring brings more repiping jobs than anticipated. Update your income on HealthCare.gov as soon as you see that change, since earning more than you reported could leave you repaying part of your credit at tax time. If business drops off instead, revise the estimate downward.
Next step: Show me last year's tax return along with your business figures for this year. We'll work out a sensible income projection before April comes around.
Now suppose his wife has no health problems. Once their income exceeds the credit range, every Kanawha County marketplace plan comes at full price. That's when I'd also quote a nationwide PPO with medical underwriting. Both spouses complete health questions, and the insurer can approve the application, approve it while excluding a previous condition, or reject it. People in good health frequently find premiums below the full marketplace rates, while gaining access to a network extending beyond West Virginia.
Next step: Give me a call with your birth years and the doctors you see. I'll compare nationwide PPO pricing with the full-price options in Kanawha County.
