Here's how plumbers and pipefitters in North Carolina are saving money on their health insurance this year

Nationwide PPO coverage remains available in North Carolina, and mountain residents can also consider marketplace PPOs without a huge price difference. Here's how I'd help High Country plumbers choose coverage around their winter workload.

Sam Jaber, licensed health insurance broker

Hi, I'm Sam Jaber. Health insurance is what I do, North Carolina is among my licensed states, and I help clients by phone. That's especially useful when plumbers are busy throughout January.

Picture a plumber living just outside Boone. Most of his annual income comes from winter, when freezing weather bursts pipes in vacation properties that haven't been visited since October. Calls keep coming until temperatures rise. Spring slows down, while summer brings renovation jobs and well pump replacements. Whether the year is profitable depends heavily on how severe the winter gets. That makes choosing health insurance tricky, although the High Country marketplace offers more flexibility than you might assume.

You can still get a nationwide PPO in North Carolina

Regardless of what you find on HealthCare.gov, I can help you obtain a nationwide PPO in North Carolina.

Unlike some places, this mountain marketplace includes PPO coverage. Watauga and Ashe counties have 17 HMOs and 9 PPOs for 2026, offered by 2 insurers. For a 40-year-old comparing silver plans in the same county before subsidies, the least expensive PPO is 15% above the lowest-priced non-PPO. Buncombe's difference is 19%. That's a manageable premium difference for some households. Private nationwide PPOs use another pricing approach: medical underwriting, where the insurer evaluates your health rather than charging a uniform rate.

Winter money and the income estimate

Premium tax credit eligibility is tied to your household's projected net self-employment earnings for the coverage year. For a plumbing business, subtract truck costs, fittings, fixtures, equipment, licensing and insurance from revenue. HealthCare.gov needs your estimated income, and you're expected to update it promptly when circumstances change.

The difficulty for mountain plumbers is predicting winter. You're estimating income in the fall without knowing whether January will deliver three serious freezes or none. A particularly harsh season can push earnings well above expectations. Once February gives you a clearer picture, revise your estimate rather than waiting for a possible tax-time repayment.

Scenario 1 of 3

In a normal year, we qualify for the credit

When your household qualifies for a premium credit, I'd look at marketplace plans first. The credit applies there, often making them the most economical choice. Watauga is unusual because the PPO premium is only 15% higher before credits than the alternatives. If out-of-network physician access matters to your family, that difference may be worthwhile. But when all your doctors participate in an HMO network, choosing the HMO can preserve the premium savings.

Scenario 2 of 3

A big winter put us over, and we're healthy

A couple of successful years or one especially busy winter can push a two-truck shop beyond premium credit eligibility. Then you're looking at full HealthCare.gov premiums. For a healthy household, I'd compare a medically underwritten nationwide PPO. The carrier reviews medical answers and can approve the policy, reject it, or approve it with one previous condition excluded. Healthy applicants may receive premiums below the marketplace's full rate. Because benefits differ among plans, I'll explain exactly what each one covers before you decide.

Scenario 3 of 3

A light winter

A warmer-than-usual winter can leave a mountain plumbing business short on revenue. North Carolina's Medicaid expansion, which began in December 2023, allows working-age adults earning up to 138% of the federal poverty line to qualify. Workers' compensation, if you have it, covers job-related injuries rather than ordinary sickness.

Where the North Carolina figures come from

  • North Carolina's individual marketplace runs on HealthCare.gov; North Carolina is one of the states in the federal 2026 QHP Landscape file Source
  • For 2026, Watauga and Ashe each have 17 marketplace HMOs and 9 PPOs from 2 insurers; Buncombe has 50 HMOs and 9 PPOs from 5 insurers Source
  • Matched within each county (silver plans, age 40, before any subsidy), the lowest-priced marketplace PPO costs 15% more than the lowest-priced plan of any other network type in Watauga and Ashe, and 19% more in Buncombe Source
  • HMO plans generally don't cover care outside their network except in an emergency, and PPO plans cover out-of-network care at a higher cost without a referral Source
  • Self-employed people report net self-employment income to the marketplace, estimate it for the coverage year, and should update it as soon as it changes Source
  • North Carolina expanded Medicaid on December 1, 2023, to adults ages 19 through 64 with income up to 138% of the federal poverty line Source
