Even though the Des Moines marketplace has no PPO, I can still help you apply for nationwide PPO coverage in Iowa. Here's how Polk County real estate agents can choose insurance based on their medical needs and the commissions they expect to earn.
Hi, I'm Sam Jaber. I'm licensed to broker health insurance in 36 states, and Iowa real estate agents can handle their coverage decisions with me over the phone.
Picture a real estate agent living in Ankeny who handles transactions across greater Des Moines. She's selling entry-level houses in Altoona, newly constructed homes in Grimes and Waukee, and older bungalows around Beaverdale. Many buyers are relocating for downtown insurance and finance positions, including some arriving from the Twin Cities or Chicago. Her spring calendar fills quickly, while winters can drag on without much activity. Since her brokerage doesn't offer group health insurance, she shops the federal marketplace each fall. But in Polk County, not one of the available plans is a PPO.
Polk County's 2026 marketplace selection contains 58 individual medical plans, including 32 EPOs and 26 HMOs. Across Iowa, one insurer offers all 12 marketplace PPOs, limited to 7 of the state's 99 counties. Every county with those PPOs is in northwestern Iowa, leaving central Iowa without that option.
I can help you obtain a nationwide PPO privately in Iowa, outside the marketplace. Medical underwriting determines the premium, and the participating provider network extends across the country. That's useful when your real estate work or personal life takes you away for conferences, family visits or winter vacations.
HealthCare.gov calculates your premium credit from expected household income during the coverage year, with assistance ending above four times the federal poverty level. For a real estate agent, we start with gross commissions, subtract the brokerage's share and account for expenses such as board dues, MLS and lockbox access, advertising, open houses and transportation.
Iowa provides expanded Medicaid through the Iowa Health and Wellness Plan. According to KFF, adults can qualify up to 138% of the poverty level. The state is also introducing a Medicaid work requirement ahead of schedule, with KFF's tracker identifying December 1, 2026 as the implementation date.
Premium credits apply only to marketplace insurance. For Polk County residents, that means choosing an HMO or EPO. Both generally restrict out-of-network coverage to emergencies. Rather than looking only at the monthly premium, we'll compare which networks include your physicians and the Des Moines hospital system you'd prefer.
Next step: Give me your doctors' names and last year's income figure. I'll compare the Polk County policies that include those physicians and show what each costs after your premium credit.
If your earnings rise above the credit range, you'll pay the entire marketplace premium for coverage centered on Iowa providers. That's when many successful agents compare a private nationwide PPO. The insurer reviews your medical answers and may approve you, approve coverage excluding a named condition, or decline the application. Healthy applicants often find premiums below unsubsidized marketplace prices, with out-of-state participating physicians treated as in network. We'll examine the benefits of each policy. And if someone in your household has a health condition, mention it when we speak. That alone doesn't eliminate the private possibility, and I'll compare both routes.
Next step: Call me, and I'll compare a nationwide PPO quote directly against the full, unsubsidized marketplace premiums available in Polk County.
When spring real estate sales stall, an agent's annual income can fall quickly. Before enrolling, check whether your projected earnings approach the poverty level. The Iowa Health and Wellness Plan may be available at or below 138%, subject to its new work requirement. As transactions resume, report the income changes to HealthCare.gov and transition to marketplace coverage. Keeping pending deals organized in one place makes those updates easier.
Next step: If you're expecting a financially difficult year, call me before renewing. We'll review your income and determine which coverage program may apply.
For coverage beginning in 2027, HealthCare.gov enrollment runs from November 1, 2026 until January 15, 2027. Selecting a plan by December 15 gives you a January 1 effective date. Because medically underwritten insurance requires approval time, start reviewing those alternatives in October.
Straight answers, no sales pitch.
No. During 2026, every marketplace plan offered in Polk County was either an HMO or an EPO.
Only 7 of Iowa's 99 counties have marketplace PPO coverage, all located in the northwest and served by one insurer.
Yes. Iowa is introducing a Medicaid work requirement, and KFF's tracker identifies December 1, 2026 as its start date.
Yes. If you're healthy, I can help you purchase a private nationwide PPO regardless of where you live in Iowa.
Tell me your Iowa county, which household members need insurance and roughly what you earned in net commissions last year. I'll compare actual pricing for both coverage routes and explain which I'd choose in your situation.
