Here's how truck drivers in Iowa are saving money on their health insurance this year

A nationwide PPO is still available in Iowa. If you're moving grain, machinery or general freight along I-80, that's important because your county's marketplace doesn't offer PPO coverage. Here's how owner-operators in western Iowa are finding plans that work for their driving schedules.

Sam Jaber, licensed health insurance broker

Hi, I'm Sam Jaber. I work exclusively with health insurance and hold an Iowa license. Whether you're waiting at a co-op grain elevator or parked at a truck stop, you can reach me by phone.

Consider a truck owner based in Council Bluffs, just across from Omaha where I-80 meets I-29. He might spend part of his week hauling grain and feed between western Iowa elevators and processing facilities, then switch to moving agricultural equipment and replacement parts. Longer deliveries take him east past Des Moines toward Chicago, west through Nebraska or south along I-29 into Kansas City. His family's regular doctors are in Council Bluffs, but when he gets sick away from home, he needs treatment wherever he's stopped.

You can still get a nationwide PPO in Iowa

Here's the first thing I'd tell him: I can arrange nationwide PPO coverage in Iowa, even when he's calling from somewhere along his route instead of home.

He won't see a PPO among his local HealthCare.gov choices. For 2026, Pottawattamie County offers 35 HMOs and 31 EPOs from 5 insurance companies, without a single PPO. Iowa marketplace PPOs are limited to seven counties in the northwest. HMOs and EPOs generally cover non-network providers only for emergencies, so an ordinary walk-in appointment for a cough in Cheyenne usually comes out of your pocket. PPOs can contribute toward those visits without referrals. Nationwide PPO policies are available privately, with pricing determined through medical underwriting.

What the truck actually clears

Your household's projected earnings for the coverage year determine eligibility for premium tax credits. If you own your truck, we're talking about net self-employment profit after deducting fuel, loan payments, tires, mechanical repairs, permits and truck insurance from your settlements. You'll provide an estimate when applying, then update it if harvest demand, freight rates or unexpected repairs change your financial picture.

Scenario 1 of 3

Long runs, healthy family

When your household income is too high for a premium credit, every marketplace plan in Pottawattamie County comes at full price, and none offers PPO coverage. I'd compare those rates with a medically underwritten nationwide PPO. The company asks about your health and can approve the application, approve it with one prior condition excluded or decline it. Healthy drivers with favorable medical histories can often pay less than the marketplace's full premium while receiving the same treatment for a clinic appointment in Wyoming or Illinois as one near home. We'll also verify your family's Council Bluffs doctors. If someone in your household has an ongoing medical condition, don't dismiss private coverage automatically. Tell me about it so I can explain both choices.

Scenario 2 of 3

Short hauls, home most nights

If your work involves moving grain between elevators and processors in western Iowa, you're probably sleeping at home most evenings. Add eligibility for a premium tax credit, and HealthCare.gov will generally be more affordable because the credit doesn't apply to private plans. With 66 HMOs and EPOs available, focus on the network your family already uses, schedule routine appointments during home time and remember that emergency coverage travels with you.

Scenario 3 of 3

A rough year

A failed engine, disappointing harvest or declining freight rates can put serious pressure on household finances. Iowa's Medicaid expansion, the Iowa Health and Wellness Plan, provides coverage for qualifying adults with income up to 138% of the federal poverty level. Occupational accident coverage handles injuries related to your job, not illnesses. Make sure you keep your settlement records available.

Where the Iowa figures come from

  • Iowa's individual marketplace runs on HealthCare.gov; Iowa is one of the states in the federal 2026 QHP Landscape file Source
  • For 2026 Iowa's marketplace has 121 individual medical plans from 6 insurers: 71 EPOs, 38 HMOs and 12 PPOs; one insurer offers the PPOs, sold only in Clay, Dickinson, Emmet, Lyon, O'Brien, Osceola and Sioux counties Source
  • For 2026, Pottawattamie County has 35 marketplace HMOs and 31 EPOs from 5 insurers, and no PPO Source
  • HMO plans generally don't cover care outside their network except in an emergency, EPO plans cover out-of-network care only in an emergency, and PPO plans cover out-of-network care at a higher cost without a referral Source
  • Self-employed people report net self-employment income to the marketplace, estimate it for the coverage year, and should update it as soon as it changes Source
  • Iowa has adopted the Medicaid expansion through the Iowa Health and Wellness Plan; adults qualify with income up to 138% of the federal poverty level Source

Questions truck drivers ask me

Straight answers, no sales pitch.

Can I buy a PPO on the marketplace in Council Bluffs?

No. Pottawattamie County offers only HMO and EPO marketplace coverage for 2026. Healthy truck drivers can still purchase a nationwide PPO through private coverage.

Will an Iowa EPO cover me in another state?

In most cases, only emergencies are covered unless the doctor or facility is part of your plan's network.

Can an Iowa owner-operator get Medicaid after a bad year?

You might qualify. Iowa's expanded Medicaid program covers eligible adults earning up to 138% of the federal poverty level.

Does every Iowa driver need a nationwide PPO?

No. If you return home most evenings and qualify for premium credits, a local HMO or EPO can often be the better deal.

Tell me how far you run

I'll need your home county, how often you're away during the month, everyone who needs insurance and approximately what your truck earned after expenses last year. Then I'll compare coverage around your schedule and explain the results clearly.
