Alabama business owners

Here's how small-business owners in Alabama are saving money on their health insurance this year

I can help Alabama small-business owners find a nationwide PPO, and Birmingham's marketplace PPO doesn't cost much more than the EPO alternatives. Let's compare how families running businesses in Shelby and Jefferson counties can decide which coverage makes sense.

Sam Jaber, licensed health insurance broker

Hi, I'm Sam Jaber. I'm licensed as a broker in 36 states, and I help Alabama business owners review family health coverage over the phone.

Consider a married couple living in Hoover who operate an LLC with two dry-cleaning shops, one near Highway 280 and another in Homewood. Their daughter attends Auburn, and their son is a teenager who was diagnosed with Type 1 diabetes several years ago. He sees an endocrinologist in Birmingham. Some years the dry cleaners generate strong profits, while other years the earnings are just sufficient. Since a group policy isn't available for only the two owners, they purchase their household's insurance through HealthCare.gov.

You can still get a nationwide PPO in Alabama

For 2026, both Jefferson County and Shelby County offer 54 individual health plans from four insurance carriers. That includes 44 EPO plans and 10 PPO plans. Birmingham's PPO premium difference is the largest in Alabama, but it's still relatively small. For an unsubsidized 40-year-old, the lowest-priced silver PPO costs just 9% more than the lowest-priced silver EPO.

I can also arrange a private nationwide PPO in Alabama. Each family member goes through individual medical underwriting, and the provider network extends beyond the local area, including where your daughter attends college and where grandparents live in another state.

How the company's year turns into a premium

When you operate an LLC, HealthCare.gov looks at household earnings, including business profit after expenses such as dry-cleaning solvent, machinery, wages for counter employees and rent on both stores. Other household income also counts. Premium assistance extends through four times the federal poverty level, and the income threshold for a four-person family is substantially higher than for someone living alone.

Alabama didn't expand Medicaid, and KFF puts the income eligibility limit for parents at only 18% of the federal poverty level. That means business owners shouldn't assume Medicaid will protect them if profits collapse. Marketplace premium assistance generally begins once income reaches 100% of the poverty threshold.

Scenario 1 of 3

The cleaners' profit keeps us in the credit range

Start by looking at HealthCare.gov, because that's the only place the premium credit can be used. The amount of assistance is calculated from a benchmark silver plan, so choosing the PPO means paying its additional 9% yourself. For this household, that extra expense could make sense. An EPO normally doesn't cover nonemergency care outside its network, which means the family needs to confirm that providers near their daughter's college participate. The son's endocrinologist matters just as much. If that Birmingham specialist accepts the PPO network, the marketplace PPO combined with the credit could be an excellent fit for this family's needs.

Scenario 2 of 3

A good year, and most of us are healthy

When household earnings move above the premium assistance threshold, everyone pays the full insurance premium. Even then, a family managing an ongoing medical condition might find ways to spend less. Private coverage reviews each person's medical history separately and can approve them, approve them with a stated condition excluded, or decline them. Your son's eligibility depends on his specific medical information. I'd compare private quotes for all four family members against a split where he is on a marketplace plan and the others use private insurance. Then we'll examine each plan's actual benefits before deciding.

If your advance premium credit exceeds what you actually qualify for, the difference reduces your tax refund. Starting in 2026, there's no maximum limit on that repayment.

Scenario 3 of 3

We're thinking about hiring a manager

Hiring someone who isn't a family member or an owner could make your company eligible for a SHOP group policy through the small-business marketplace. Whether that arrangement makes sense depends on the new employee and the coverage your family already carries.

Where the Alabama figures come from

  • Alabama residents use HealthCare.gov for marketplace coverage Source
  • Jefferson and Shelby counties each have 54 individual medical plans from 4 insurers for 2026: 44 EPOs and 10 PPOs Source
  • In Jefferson and Shelby counties, the lowest-priced silver PPO costs 9% more than the lowest-priced silver EPO (age 40, before any subsidy), the largest such gap in Alabama Source
  • EPOs generally cover care outside their network only in an emergency; a PPO covers out-of-network care at a higher cost to you Source
  • The premium tax credit is based on the second-lowest-cost silver plan and generally covers households with income from 100% to 400% of the federal poverty level Source
  • Alabama has not expanded Medicaid; KFF lists the parent limit at 18% of the federal poverty level Source
  • Self-employed people with no employees use the individual marketplace; a business with at least one employee who is not an owner or family member may qualify for SHOP Source
  • There is no cap on repaying excess advance premium tax credit for tax years after 2025 Source

Questions small-business owners ask me

Straight answers, no sales pitch.

Is the marketplace PPO in Birmingham much more expensive than an EPO?

Only slightly. The lowest-priced silver PPO in Jefferson and Shelby counties is 9% more expensive than the lowest-priced silver EPO.

Can one family be split between a marketplace plan and a private one?

Yes. Different family members may enroll in different plans, and marketplace insurers cannot increase premiums based on someone's health.

Will an Alabama EPO cover my college student out of town?

Usually not, unless it's emergency treatment. Otherwise, the doctor or facility generally needs to participate in the EPO network.

Can an Alabama business owner get a nationwide PPO?

Yes. Provided you're in good health, I can help arrange private nationwide PPO coverage for you and your household.
