Here's how real estate investors in Louisiana are saving money on their health insurance this year

I can help Louisiana investors purchase a nationwide PPO privately. Around Lake Charles, rental earnings change with industrial construction activity, so let's look at how your coverage should reflect a busy year, a slow one or a sale.

Sam Jaber, licensed health insurance broker

Hi, I'm Sam Jaber. I'm licensed in 36 states, including Louisiana, and I arrange health insurance by phone for landlords and real estate investors throughout the state.

Consider a Sulphur property owner with rental houses and two duplexes spread across Sulphur, Westlake and Lake Charles. During major industrial construction projects, furnished units fill with work crews and rental prices increase. When those jobs finish, tenants leave and rents decline. She's considering selling both duplexes while demand remains strong. She purchases insurance through HealthCare.gov, and her best coverage choice depends on how the coming year develops.

You can still get a nationwide PPO in Louisiana

For 2026, Calcasieu Parish offers 44 individual health plans, including 16 EPOs, 13 HMOs, 8 PPOs and 7 POS options. Before subsidies, a 40-year-old pays 54% more for the lowest-priced silver PPO than for the lowest-priced silver HMO. That PPO also costs 35% more than the silver offering from its insurer's related HMO company.

I can also set you up with a privately underwritten nationwide PPO in Louisiana. Its network extends to Houston, other states where your relatives live and destinations you visit.

Three kinds of years

HealthCare.gov looks at rental profit rather than gross rent. That means subtracting mortgage interest, Gulf Coast insurance expenses, maintenance, furnishings used for crew housing, management fees and depreciation, then adding other household earnings. Premium assistance is available through four times the federal poverty level, but ends above that threshold.

When construction activity is strong, higher rents may increase your income. Selling a property can raise it further because the gain generally counts in modified adjusted gross income, which determines your credit. During a slow year, vacancies and a roof replacement can reduce reported earnings instead. Louisiana's Medicaid expansion reaches adults earning up to 138% of the poverty level. Your coverage should reflect whichever year you're expecting.

Scenario 1 of 3

A steady year in the credit range

To receive the premium credit, you must enroll through HealthCare.gov. Since that credit is calculated using a benchmark silver plan, you absorb the PPO's 54% price difference. Many local investors therefore benefit more from an HMO, EPO or POS plan that covers their physicians. HMOs usually limit non-network benefits to emergencies, so verify coverage carefully if your specialists are in Houston.

Scenario 2 of 3

I'm selling the duplexes and I'm healthy

Profit from a property sale could take your earnings well above the credit threshold. If you've used advance premium credits, higher income means reconciling the excess when filing taxes. Beginning with tax year 2026, repayment has no cap. Let's review your insurance before advertising the property.

When you're paying the entire premium, a privately issued nationwide PPO can be attractive if you're healthy. The insurer evaluates your medical information and may approve you, approve coverage excluding a condition or decline the application. Healthy applicants often receive prices below full marketplace premiums. We'll examine the benefits together before you decide.

Scenario 3 of 3

Vacancies and repairs made a thin year

Rental profit can fall sharply when construction jobs finish and properties remain vacant. If your realistic household income projection reaches 138% of the poverty level or less, Louisiana Medicaid might be appropriate temporarily. Report higher earnings when tenants return and transition to marketplace coverage.
