Here's how real estate investors in Illinois are saving money on their health insurance this year

I can still arrange nationwide PPO coverage in Illinois. Here's how Moline and Rock Island landlords with properties and doctors across the Mississippi compare private insurance with Get Covered Illinois.

Sam Jaber, licensed health insurance broker

Hi, I'm Sam Jaber. I hold broker licenses in 36 states, and I work with Illinois landlords and property investors to arrange health coverage entirely by phone.

Imagine a Moline investor managing a dozen rental units herself. Her portfolio includes Rock Island duplexes, a small Moline apartment building and three houses across the river in Davenport and Bettendorf. She still visits a family physician in Davenport because that clinic was nearest when she first moved. Since she files in Illinois, she shops through Get Covered Illinois. Each year, her insurance decision comes down to whether she sells property and whether the available networks cover her Iowa doctor.

You can still get a nationwide PPO in Illinois

For 2026, Rock Island County residents can choose among 21 Get Covered Illinois individual medical plans from two companies: 11 HMOs and 10 PPOs. Premiums follow Illinois rating areas. In Rock Island's area, a 40-year-old without subsidies pays 23% more for the lowest-priced silver PPO than for the lowest-priced silver HMO.

I can also help you buy a medically underwritten nationwide PPO privately in Illinois. Its provider network crosses the Mississippi, so having a doctor on the Iowa side isn't an obstacle.

Holding years and selling years

Get Covered Illinois includes rental profit in household earnings after deducting mortgage interest, property taxes, insurance, maintenance, tenant turnover and depreciation. Other household income counts as well. Premium assistance extends to four times the federal poverty level.

A property closing can change your eligibility substantially. Premium credits depend on modified adjusted gross income, which generally includes the gain from selling a duplex. We therefore need to plan insurance around the upcoming year's earnings rather than simply repeating last year's figures.

Scenario 1 of 3

A holding year that qualifies for the credit

Start with Get Covered Illinois because marketplace credits can't be used elsewhere. Before comparing premiums, verify provider networks. If neither company's HMO includes your Davenport doctor, routine visits there generally won't be covered. A PPO contributes toward out-of-network treatment, although you pay more. Your credit is tied to a benchmark silver plan, leaving you responsible for the PPO's additional 23%. For someone receiving medical care in Iowa, that flexibility could justify the difference.

Scenario 2 of 3

I'm selling this year and I'm healthy

Selling can move household income well beyond credit eligibility. If you've already used advance premium credits, any excess must be reconciled on your tax return. For tax years after 2025, there's no maximum repayment limit. Account for a potential closing before selecting coverage.

When you're paying the full premium, private insurance often offers better value for healthy investors. The company reviews your medical information and can approve, decline or approve with one specified condition excluded. People in good health frequently receive premiums below unsubsidized marketplace rates. The nationwide network also covers providers in both states and elsewhere. We'll compare benefits carefully because each policy works differently.

Scenario 3 of 3

Vacancies and repairs made it a thin year

Vacancies and substantial repair expenses can bring your rental profits down sharply. Illinois has expanded Medicaid, with KFF reporting adult eligibility through 138% of the federal poverty level. If your projected household income qualifies, Medicaid might make sense temporarily. Once rental earnings improve, you can return to marketplace coverage.

Where the Illinois figures come from

  • Illinois residents buy marketplace plans through Get Covered Illinois, a state-based marketplace from plan year 2026 Source
  • Rock Island County has 21 individual medical plans on Get Covered Illinois for 2026 from 2 insurers: 11 HMOs and 10 PPOs Source
  • In the rating area that includes Rock Island County, the lowest-priced silver PPO costs 23% more than the lowest-priced silver HMO (age 40, before any subsidy) Source
  • HMOs generally cover care outside their network only in an emergency; a PPO covers out-of-network care at a higher cost to you Source
  • Net rental income counts as household income for marketplace savings Source
  • The premium tax credit uses modified adjusted gross income, which starts from adjusted gross income, so a capital gain generally raises it for that year; the credit generally covers incomes from 100% to 400% of the federal poverty level Source
  • There is no cap on repaying excess advance premium tax credit for tax years after 2025 Source
  • Illinois expanded Medicaid; KFF lists the adult income limit at 138% of the federal poverty level Source

Questions real estate investors ask me

Straight answers, no sales pitch.

Does Get Covered Illinois sell a PPO in the Quad Cities?

Yes. One company offers all ten PPOs, and Rock Island County's lowest-priced silver PPO carries a 23% premium over the lowest-priced silver HMO.

Will my Illinois marketplace plan cover a doctor in Davenport, Iowa?

Your doctor needs to participate in the network unless you have a PPO that pays for outside providers, with higher costs to you.

Does selling a rental property affect my Illinois marketplace credit?

Generally, yes. Taxable gains from selling property usually increase the modified adjusted gross income used to calculate premium credits.

Can a Quad Cities landlord get a nationwide PPO?

Yes. I can arrange private nationwide PPO coverage for applicants who meet the health requirements.

Is this a holding year or a selling year?

Share your residential county, everyone needing coverage, last year's Schedule E and any upcoming property sale. I'll compare plans for your expected income and provide prices.
