Here's how heating and cooling technicians in Illinois are saving money on their health insurance this year

A nationwide PPO is still available in Illinois. But if you're running a heating and cooling company near Peoria, don't overlook the marketplace PPO. Its price is close enough to the HMO to make comparing them worthwhile.

Sam Jaber, licensed health insurance broker

Hi, I'm Sam. Health insurance is the only type of coverage I broker, and Illinois is one of the states where I'm licensed. We can take care of everything by phone.

Picture yourself running an East Peoria HVAC company with three vans serving customers across the river, including Peoria Heights, Morton, Pekin and Washington. Cold, lengthy winters keep the heating business strong, while summer AC jobs keep the crew occupied. You've been paying for your own medical coverage for years, usually choosing an HMO without comparing much. But the marketplace around Peoria works differently from most areas, and it's worth giving your options another look.

You can still get a nationwide PPO in Illinois

I can help you get a nationwide PPO anywhere in Illinois. Still, around Peoria, the marketplace PPO deserves serious consideration before you go that route.

For 2026, one insurer provides every individual medical policy on Get Covered Illinois in Peoria, Tazewell, Woodford, Marshall and Stark counties. Its offerings include both PPOs and HMOs. CMS rating areas determine Illinois's filed rates, and Rating Area 7, covering Peoria, lists 9 HMOs and 10 PPOs. Compare the lowest silver premiums for a 40-year-old without subsidies: the PPO is only 14% higher than the HMO, Illinois's smallest difference. HMOs generally limit out-of-network benefits to emergencies. PPOs also cover outside providers, although your costs are higher, and specialist referrals aren't required.

Cold-season income and your estimate

Your premium credit is tied to household earnings during the year you're buying coverage. For an HVAC business, we use net self-employment income, subtracting costs such as equipment, refrigerant, parts, vehicles, licensing and insurance from revenue. You'll project that amount when enrolling and adjust it if business changes.

Suppose a January freeze sends all your vans out on emergency heating repairs and your revenue climbs beyond expectations. Update your income projection when you recognize the increase. That reduces future monthly credits rather than leaving the adjustment for your tax return.

Scenario 1 of 3

We get the credit and I'd like the PPO

When the PPO premium starts just 14% higher than the HMO before credits, eligible families may gain out-of-network benefits for a relatively small monthly difference. That flexibility could matter if you want a specialist in Chicago or St. Louis who isn't in your network, or prefer having more choices. On the other hand, if the HMO already includes every doctor your household uses, staying with it could save you money.

Scenario 2 of 3

The shop's grown and we're healthy

An HVAC company with three vans and steady maintenance contracts will often bring its owner's household above the income limit for credits. At that point, marketplace HMOs and PPOs both carry their full premiums. If your family is healthy, we should also compare a nationwide PPO with medical underwriting. The insurer reviews your health questionnaire and can accept you, decline you, or accept you while excluding one previous condition. Favorable health can produce a lower premium than the marketplace's full price, with a network extending beyond Illinois. I'll explain the different benefit terms before you choose.

Scenario 3 of 3

A thin year

When winter temperatures stay mild, heating revenue can fall short. If your household has a particularly low-income year, Illinois's expanded Medicaid program covers qualifying adults earning no more than 138% of the federal poverty level.

Where the Illinois figures come from

  • Illinois runs its own state-based marketplace, Get Covered Illinois, rather than HealthCare.gov Source
  • In Peoria, Tazewell, Woodford, Marshall and Stark counties, the individual medical plans on Get Covered Illinois for 2026 are HMOs and PPOs from a single insurer Source
  • In CMS's Illinois geographic rating areas, Rating Area 7 is Fulton, Knox, LaSalle, Marshall, McDonough, Peoria, Putnam, Stark, Tazewell and Woodford counties Source
  • Rating Area 7 has 2026 rates filed for 9 HMOs and 10 PPOs from one insurer; compared within each rating area (silver plans, age 40, before any subsidy), the lowest-priced PPO costs 14% more than the lowest-priced HMO in Rating Area 7, the smallest gap of the 11 Illinois rating areas that have both Source
  • HMO plans generally don't cover care outside their network except in an emergency, and PPO plans cover out-of-network care at a higher cost without a referral Source
  • Self-employed people report net self-employment income to the marketplace, estimate it for the coverage year, and should update it as soon as it changes Source
  • Illinois has adopted the Medicaid expansion; adults qualify with income up to 138% of the federal poverty level Source
