Here's how general contractors and builders in Nebraska are saving money on their health insurance this year

Nationwide PPO coverage remains available in Nebraska, even though the marketplace serving Papillion and Gretna doesn't offer it. Here's how builders throughout Sarpy County can approach insurance for their families.

Sam Jaber, licensed health insurance broker

Hi, I'm Sam Jaber. Health insurance brokerage is my business, I'm licensed in Nebraska and work entirely by phone. You can even call me while parked in your truck at a Gretna construction lot.

Consider a Papillion contractor building custom houses in subdivisions expanding south and west of Omaha. Between home projects, he takes on basement finishing and deck work while framing crews wait for inspections. His wife manages estimates and bookkeeping. They have two school-age children whose club volleyball tournaments take them to Des Moines and Kansas City, plus an annual summer trip to the Colorado mountains. Business has been profitable, and everyone in the household has enjoyed good health.

You can still get a nationwide PPO in Nebraska

The first thing to know is that your Sarpy County family can still get nationwide PPO coverage in Nebraska. I can help you set it up.

Sarpy County's marketplace doesn't offer a PPO. Its 2026 selection includes 81 medical policies from 5 insurers, and every one is an EPO. Nebraska does have 9 marketplace PPOs from one insurer, but they're sold in only 34 of its 93 counties, primarily in western Nebraska, not the Omaha area. EPOs generally exclude out-of-network care except emergencies. That means a sprained ankle treated at a Des Moines volleyball tournament or a sinus infection visit in Breckenridge could become your expense if the clinic isn't in network. Private nationwide PPO coverage, priced after medical underwriting, is designed for families who travel like this.

The question that sorts it out

The deciding factor is your household's expected income for the coverage year. HealthCare.gov looks at a contractor's net earnings, not the total value of signed jobs. Deduct subcontractors, lumber, concrete, equipment payments, builder's risk insurance and trucks to calculate profit. You'll project that amount for the coming year and revise it if a large contract arrives or an existing project gets delayed.

Enrollment for 2027 coverage opens November 1 and ends January 15. To begin coverage on January 1, you'll need to enroll no later than December 15. Construction often slows after the final foundations are poured and before frozen ground makes more work difficult, so that period can be a convenient time for us to talk.

Scenario 1 of 2

The company netted well and we're all healthy

Think back to the family in our example. If their earnings exceed the marketplace tax credit limits, all 81 Sarpy County EPOs carry their full premiums. Without that credit advantage, HealthCare.gov may no longer be the obvious choice. I'd recommend getting an actual quote for a medically underwritten nationwide PPO before deciding.

Medical underwriting starts with health questions for everyone applying. The insurer may accept the family, reject the application or approve coverage while excluding a specific previous condition. Healthy households can sometimes receive premiums below full marketplace prices through that process. Their nationwide network can also follow them to Des Moines, Kansas City and ski destinations. Before you enroll, I'll verify whether your family physician and pediatrician in Papillion participate.

Scenario 2 of 2

A slow year puts us back in credit range

Construction income isn't always predictable. Higher rates may cause customers to abandon building projects, reducing your annual profit. If that brings household earnings within the premium tax credit limits, HealthCare.gov will usually offer the lower-cost route because those credits apply only to marketplace policies.

Because Sarpy County offers EPOs exclusively, we'll focus on which networks include the hospital and doctors your family already uses around Omaha and Sarpy County. Coverage networks aren't all the same, so we'll verify individual providers. If your children regularly travel for sports, we'll also check network availability in Kansas and Iowa.

Where the Nebraska figures come from

  • Nebraska residents buy marketplace coverage on HealthCare.gov Source
  • For 2026, Sarpy County has 81 individual medical plans on the marketplace from 5 insurance companies, and every one of them is an EPO Source
  • Nebraska's marketplace sells 164 individual medical plans for 2026: 155 EPOs and 9 PPOs; all 9 PPOs come from a single insurance company and are offered in 34 of the 93 counties, not including Sarpy or Douglas Source
  • An EPO covers services only from doctors, specialists and hospitals in its network, except in an emergency; a PPO lets you see providers outside the network without a referral, at an extra cost Source
  • Marketplace savings are based on your estimated net self-employment income for the coverage year, which you update when your business changes Source
  • Open enrollment for 2027 coverage starts November 1; December 15 is the last day to enroll for coverage starting January 1, and open enrollment ends January 15 Source

Questions general contractors and builders ask me

Straight answers, no sales pitch.

Is there a marketplace PPO in Papillion or Gretna?

No. Every one of Sarpy County's 81 marketplace medical policies for 2026 is an EPO.

Will a Sarpy County EPO cover my kid at an urgent care in Iowa?

Coverage applies when the clinic participates in your plan's network or the visit involves a genuine emergency.

Can a Nebraska contractor buy a nationwide PPO off the marketplace?

Yes. Nationwide PPO coverage is available privately, with pricing determined after medical underwriting. I can provide a quote over the phone.

When can I change my family's plan for next year?

The enrollment period runs November 1 through January 15. December 15 is the deadline if you need coverage effective January 1.

Send me the job board, not the blueprints

Let me know your county, everyone needing coverage, your regular doctors and approximately how much profit the business made last year. I'll compare Sarpy County EPO premiums with a nationwide PPO and tell you what I'd choose.
